Sugar substitutes are linked to worse blood-sugar control

Researchers at Tufts University reviewed the evidence and ran a new meta-analysis of 21 randomized trials, published in Current Atherosclerosis Reports. Choose a diet soda over the full-sugar version and it feels like the careful choice. No sugar, no calories, nothing for your body to deal with. That last assumption is the one a new research review picks apart.

The review comes from a team at Tufts University and appears in the journal Current Atherosclerosis Reports. Pulling together current scientific evidence from trials and long-term studies, the authors reach a wary conclusion: these sugar substitutes have real effects on the body, and may work against your blood-sugar control.

What counts as a sugar substitute

Non-nutritive sweeteners are the calorie-free or low-calorie stand-ins for sugar. They have become widespread in the food supply, turning up in diet drinks, sugar-free yogurt and protein bars. The common ones include saccharin, sucralose, stevia and the aspartame in most diet sodas.

Why the trial evidence is the new part

Concern about these sweeteners is not new. Large studies that follow people over years have tied total non-nutritive sweetener intake, and each commonly used one, to a higher risk of type 2 diabetes, along with some heart problems. Those results are consistent with prior evidence on sweetened drinks. The catch in that kind of research is easy to name once you see it: someone already sliding toward diabetes often switches to these products, so the sweetener can look like a cause when it is really a marker of existing risk. The researchers call this reverse causation, people at higher cardiometabolic risk choosing to use these products.

What sets this review apart is how the team handled that trap. Alongside the survey data, they pooled trials that compared sweeteners against non-caloric controls like water or placebo, rather than against sugar. Meng Wang, a researcher on the team, said that by focusing on non-caloric comparators, “we better isolated the direct physiological effects of the sweeteners themselves, not the calories they replace.” Take the calories out of the comparison, and the sweetener still did something on its own.

The blood-sugar signal, and the gut

When the team pooled those trials, a consistent pattern showed up. Compared with water or a placebo, the sweeteners raised fasting insulin and a marker of long-term blood sugar, and nudged insulin sensitivity the wrong way. These were small shifts in how the body handles sugar, well short of a diagnosis, but real.

Why would something with no calories touch blood sugar at all? The authors point to the gut. They suspect the sweeteners work through changes in the composition and function of the gut microbiome, the community of bacteria in your intestines that helps set how you process food. They also cite a recent trial that used a human-to-mice microbiota transplant, moving gut bacteria from people into mice, to test what happened to blood sugar. For now the microbiome route is a leading explanation that still needs direct testing.

The takeaway for sugar-free products

The evidence still stops short of proof. The paper pools existing studies, and much of the underlying data comes from watching what people eat and drink. The trial results describe short-term harmful effects on blood sugar, a signal at the level of markers. Given the potential for harm, caution is warranted, the authors write.

Health authorities have been drifting the same way. Back in 2023, the World Health Organization advised against using non-sugar sweeteners to control body weight or to reduce the risk of noncommunicable diseases, pointing to possible long-term harms including a higher risk of type 2 diabetes.

That leaves a practical question for anyone holding a diet drink. The study’s senior author, Tufts cardiologist Dariush Mozaffarian, was measured about what to do with the finding. Low-calorie sweeteners “may be a better alternative” to large amounts of added sugar, “such as in multiple servings of soda,” Mozaffarian said, but “we can’t simply assume they are safe and innocuous, and avoiding them whenever possible appears a prudent choice.”

Water and whole foods stay the safer default, and the sugar-free aisle earns more skepticism than it usually gets.
