Does fermented food fight inflammation? A trial says mostly no

A randomized crossover trial in 87 healthy adults, published in the European Journal of Clinical Nutrition, found daily sauerkraut did little for inflammation or the gut barrier over four weeks. The one signal, a small drop in blood pressure, showed up even when the bacteria were dead.

Sauerkraut, kimchi and kombucha wear a health halo. Eat more fermented food, the pitch goes, and you will lower inflammation and heal your gut. Fermented foods have been proposed to improve blood sugar and dampen the body’s inflammatory response, partly by strengthening the gut barrier.

The pitch is everywhere. Until now, the scientific evidence for their health benefits had stayed scarce, and few trials had tested the idea head-on. So a team reporting in the European Journal of Clinical Nutrition fed sauerkraut to volunteers for a month and watched the markers.

The result was a letdown for the hype. Over four weeks, daily sauerkraut was not linked to an appreciable health benefit in these healthy adults. The one real change was a small reduction in systolic blood pressure, the top number in a reading, and it appeared even when the sauerkraut had been pasteurized, its live cultures killed by heat.

What the trial actually put to the test

The study was a randomized crossover intervention trial, a strong way to test a food. Each of the 87 healthy adults ate both kinds, fresh and pasteurized sauerkraut, so every person served as their own comparison. Before each stretch, they spent four weeks during which no fermented foods were eaten, a washout to clear the slate. They then ate a daily portion for a month, and the team compared each person’s readings before and after each stint.

That design does real work. Because each person is measured against their own baseline, a crossover trial can catch a genuine change with fewer people than a study that pits one group against another. The trade-off is who was studied and for how long: well people, a month per food.

The dead bacteria worked too

The theory was that live cultures do their work partly by strengthening the gut barrier. The data told a different story. The sauerkraut produced no change in gut barrier integrity, the intestinal lining that is meant to keep unwanted material out of the bloodstream.

Markers of inflammation and blood sugar barely moved, and the few modest changes only surfaced after the team split people up by age and usual fiber intake. Slice a group finely enough and small differences appear; on their own, they are weak evidence.

Only one effect held up. Both fresh and pasteurized sauerkraut were tied to a small reduction in systolic blood pressure, the top number in a reading. That is the awkward part for the probiotic story. If the pasteurized version, its bacteria killed by heat, lowered blood pressure just as well, then the live bacteria are not essential to the effect. Whatever nudged the numbers down, it was probably not the live cultures the marketing leans on.

Is sauerkraut still worth eating?

This is not a reason to give up sauerkraut. It is still a cheap, fiber-rich vegetable, and nothing here suggests it is bad for you.

That verdict has boundaries. It rests on a single stretch of four weeks of daily sauerkraut, in people who were healthy to begin with, and it measured blood markers, not disease. A null result there does not prove fermented foods do nothing. The effect could differ over months instead of weeks, in people who are older or already unwell, or on outcomes this study never measured. Fermented foods remain an under-studied corner of nutrition, and one trial rarely settles a question.

For now, sauerkraut looks like a perfectly good vegetable carrying a health reputation its own evidence has yet to earn.
