Heart attack patients carried more microplastics in their blood
Researchers in Rome and Verona measured plastic particles in the coronary blood of 61 patients having angiography. The particles turned up far more often in heart attack patients, though a smoking history predicted their presence better than the heart attack itself.

Plastic shows up in the ocean, in bottled water, and in the food on the table. A small study from Italy now reports it somewhere more unsettling: inside the blood in the arteries that feed the heart. People treated for a serious heart attack were far more likely to have micro and nanoplastics in that blood than people with healthier arteries, according to research published in the European Heart Journal.
The work comes from teams at Sapienza University of Rome, the University of Verona, and the University of Campania Luigi Vanvitelli in Naples. Until this study, little was known about whether these particles reach the coronary circulation, the blood in the arteries that supply the heart, or whether smoking and air pollution affect their presence.
The result is striking, and easy to over-read. This was a small snapshot, not a study that tracks people over time, and its own analysis put a familiar exposure at the center: cigarette smoke.
What micro and nanoplastics are
Micro and nanoplastics are tiny plastic fragments, most far too small to see, that come from larger plastic waste breaking down over years. They are found virtually everywhere in the environment, in the air, water, and food. Scientists have started to detect these particles in human tissues and organs, including blood. The most common type found in this study was polyethylene, the plastic used in packaging and consumer products.
What the study actually measured
Researchers enrolled 61 patients undergoing coronary angiography, a procedure that threads a thin tube into the arteries of the heart to check for blockages. Each patient had one of three diagnoses: a heart attack, chronic ischemic heart disease, which is stable, long-term heart disease, or normal coronary arteries. The team drew blood from the vessels supplying the heart and from elsewhere in the body, then measured the plastic particles in it.
Among the heart attack patients, plastics showed up in about 84 percent of cases, compared with 40 percent of those with long-term disease and 32 percent of those with normal coronary arteries. The heart attack group also carried a wider variety of plastic types. The particles appeared in both the coronary blood and blood drawn from elsewhere, with the highest concentrations in coronary blood.
Where smoking clouds the picture
The design is the catch. Because researchers measured plastics, smoking, and pollution at a single moment, the study can show that these things occur together, without showing that any one of them causes another. A snapshot cannot untangle what came first.
When the team asked which factor best explained who had plastics in their blood, one answer stood out, and it was smoking. A smoking history was the only independent predictor, meaning the one thing that still mattered after everything else was accounted for. Smokers were about six times more likely to have microplastics in their blood.
The plastics kept other bad company, too. Patients with more long-term exposure to polluted air were also more likely to have microplastics in their blood, and every patient who both smoked and breathed dirtier air had plastics detected, compared with only 12.5 percent of those with neither. The heart attack patients also showed higher levels of inflammatory markers, a sign of an immune system under strain. The researchers suspect smoking may make it easier for the particles to enter the bloodstream through the lungs.
That points to a plausible reading: the plastics may be riding along with smoking and pollution rather than driving the damage on their own. As Emanuele Barbato, who led the research, put it: “These findings do not prove that microplastics cause heart attacks, but they reveal a strong association between environmental exposures, microplastics in the blood and cardiovascular disease.”
What it means for you
This is not the first hint that plastics and heart trouble may be connected. In an earlier study of people having fatty plaque scraped out of a neck artery, researchers found plastic particles inside the plaque, and their presence was associated with an increased risk of heart attack, stroke, and death. The new work adds a look at the coronary blood itself, though it remains early, small, and unable to prove cause.
For a reader, there is no action to take from this study alone. What it flags most clearly are exposures already known to be hard on the heart, tobacco smoke and dirty air, and those stay worth avoiding whatever plastics turn out to do. For now it is a reason to fund better studies and keep an eye on the question.
The most solid signal here is a familiar one: smoking and polluted air are bad for the heart, and this study is one more nudge to take both seriously.
