Loneliness was tied to a higher risk of heart valve disease

Across 462,917 UK Biobank adults tracked for about 14 years, those who felt lonely had a higher risk of degenerative heart valve disease. Living alone or being socially isolated did not carry the same risk.

Doctors have spent decades mapping what wears out the heart, from high blood pressure to cholesterol. How connected a person feels has drawn far less notice. Until now, the role of loneliness in heart valve disease had been largely overlooked.

A large study in the Journal of the American Heart Association has now taken a look. Researchers at the Second Xiangya Hospital of Central South University, in China, drew on nearly 463,000 adults in a long-term British health project. Those who said they felt lonely were more likely to develop degenerative valvular heart disease, a slow stiffening or leaking of the heart’s valves.

Which kind of being alone mattered

The analysis pulled apart two things we tend to lump together. One is loneliness, the private feeling that your relationships fall short of what you want. The other is social isolation, the outward fact of living alone or rarely seeing anyone.

Only the feeling was linked to the valves. Loneliness, rather than social isolation, was tied to the higher risk. Social isolation on its own, measured as living alone, minimal contact, or no social activities, showed no clear link to the disease.

An American Heart Association expert who was not part of the study put it plainly. “Loneliness is about the quality of relationships, not quantity,” said Crystal Wiley Cene, a professor of medicine and public health at the University of California, San Diego. You can feel it in a crowd, she noted, when the connections there are thin.

What 462,917 UK Biobank records showed

The strength here is size and time. The work was a prospective study, meaning it tracked healthy people forward for years and watched who fell ill, and it involved 462,917 participants from the UK Biobank. Over roughly 14 years, more than 11,000 of them developed degenerative valvular heart disease. The researchers focused on two forms: aortic valve stenosis and mitral valve regurgitation, a stiffened valve and a leaky one.

Numbers that large make a faint signal easier to trust. Adults who reported the most loneliness had about a 19 percent higher risk of degenerative valve disease than those who reported none. That is a small, steady increase, and it showed up across both of the main valve problems.

What might tie loneliness to the valves

Why would a feeling reach the heart’s valves? Part of the answer looks like plain behavior. When the researchers accounted for habits like smoking, heavy drinking and inactivity, those unhealthy patterns explained part of the association between loneliness and valve disease. Lonely people, on average, live a little less healthily, and that seems to carry some of the risk.

Genes told a separate story. The link between loneliness and valve disease held regardless of a person’s inherited risk, and it was strongest in people who had both high genetic risk and deep loneliness. That points to loneliness acting as a factor in its own right, separate from the genes a person is born with.

Valve disease is also becoming more common as people live longer. It is a growing public health concern in aging societies, which is part of why the team went looking for risks that a person might actually be able to change.

What the loneliness finding cannot show

The limits are the ones that come with this kind of research. This was an observational study built on existing UK Biobank data, so it can reveal a link but not prove that loneliness harms the valves. The direction could even run backward, since early, undiagnosed heart trouble might make someone withdraw and feel lonelier. Loneliness was also measured just once, with a questionnaire, and most UK Biobank participants are white adults, so the numbers may not carry to everyone.

Even so, the researchers see something useful in a risk that is not fixed in place. “Our findings suggest that loneliness may be an independent and potentially modifiable risk factor for degenerative valvular heart disease,” said Zhaowei Zhu, a cardiologist at the Second Xiangya Hospital who helped lead the work. Blood pressure and cholesterol already get checked as a matter of routine; this is a case for asking about social connection with something like the same seriousness.

For now it is only a link, but it makes a real case for taking loneliness as seriously as the heart’s better-known risks.

If it really is modifiable, something has to do the modifying. That is the premise behind social prescribing, which is now being tested in its own right.
