Walking breaks every 30 minutes worked best. Nobody kept it up.

A 19,342-person trial tested 5-minute walking breaks at three frequencies. The most effective schedule was the least sustainable, and hourly landed in between.

Advice to get up and walk every half hour is now standard, and almost nobody follows it. A study in the British Journal of Sports Medicine tested three different schedules on 19,342 people and found the reason built into the results: the schedule that worked best was the one people found hardest.

The authors frame the gap precisely. Brief, regular movement breaks have been proposed as a public health strategy to offset the harms of prolonged sedentary behavior; how viable they are in the real world is unclear.

A trial in ordinary conditions

This was not a laboratory study. Adults enrolled in a 2-week pragmatic intervention embedded within an interactive podcast, and the drop-off before anything began is itself informative: of 19,342 enrolled, 59.4% (11,484) initiated the intervention.

Four in ten signed up and never started. Any real-world program carries that tax, and it usually goes unreported.

Those who did start took 5-minute walking breaks at their self-selected break frequency, across three arms: every 30, 60 or 120 minutes. Fatigue, positive affect and negative affect were assessed pre-intervention and post-intervention.

Everything was viable, some things more than others

All arms exceeded the viability threshold for implementation outcomes, so none of the schedules was rejected outright.

But feasibility separated them, and in the direction you would guess. Feasibility was higher at lower break frequencies: 3.41 for every 30 minutes, 3.80 for hourly, 4.01 for every two hours.

Acceptability and appropriateness stayed high across the board, at roughly 3.9 to 4.0 everywhere. People thought the idea was sensible regardless of frequency. What differed was whether they could actually fit it in.

The benefits ran the opposite way

Reported fatigue and negative affect decreased and reported positive affect increased significantly across all arms. Everyone felt better, at every frequency.

The size of the improvement, though, tracked how often people walked. Improvements showed a dose-response pattern. Fatigue fell by 1.55 in the 30-minute arm, 1.41 hourly and 1.19 at two-hourly intervals. Positive mood rose by 1.86, 1.65 and 1.27 respectively.

The study also checked those against thresholds for what counts as a change a person would notice. The 30-minute and 60-minute arms exceeded minimal important difference thresholds for fatigue and positive affect, while only the 30-minute arm exceeded the threshold for negative affect.

So the most frequent schedule was the only one to clear the bar on all three measures. It was also the one people found hardest to sustain.

The trade-off, stated

That tension is the paper’s actual contribution, and the authors resolve it explicitly: hourly breaks offered the best balance between feasibility and effectiveness.

Hourly is not optimal on either axis. It is second best on both, which in a behavior nobody is obliged to perform is usually what wins.

What this does not show

Two weeks, and every outcome self-reported. Fatigue and mood are legitimate endpoints, and they are also the ones most susceptible to expecting an effect, in a study where nobody could be blinded to which arm they were in.

Nothing physical was measured. No blood sugar, no blood pressure, no body composition. The harms of prolonged sitting that the intervention is meant to offset are metabolic and cardiovascular, and this study did not test whether the breaks touch them.

Recruitment through a podcast also selects for a particular person: engaged, interested in health, willing to try something. Feasibility in that group is an optimistic estimate of feasibility anywhere else.

Why it is useful anyway

Most sedentary-behaviour research establishes that sitting is bad and stops. This one asked what people will actually do about it, at scale, without supervision, and got an answer with a number attached.

The finding that four in ten never started is arguably as valuable as the dose-response. Advice only works if it survives contact with a working day.

What problem are movement breaks meant to solve?

Long unbroken periods of sitting are associated with worse metabolic and cardiovascular outcomes, and that association survives adjustment for how much someone exercises overall. In other words, a daily workout does not appear to fully offset eight hours in a chair. Breaking up the sitting is the proposed fix, and short frequent walks are the most commonly suggested form.

What does a pragmatic trial mean?

One run in real conditions rather than a laboratory. Here that meant recruiting through an interactive podcast and letting people fit the breaks around ordinary life, with no supervision. The trade-off is deliberate: you lose control and precision, and you gain a realistic picture of whether people can actually do the thing. A 40% drop-off before starting is itself a finding.

Why is feasibility highest at the least effective frequency?

Because interrupting work every 30 minutes is disruptive. A five-minute walk twelve times across a working day is an hour of walking and, more to the point, twelve interruptions to concentration. Every two hours is easy to accommodate and correspondingly does less. That inverse relationship is the practical tension the study surfaces, and it is why the authors land on hourly.

How meaningful were the mood and fatigue changes?

The study compared them against minimal important difference thresholds, which are pre-set values for how much a score has to move before it matters to a person rather than merely to a statistician. The 30- and 60-minute arms cleared that bar for fatigue and positive mood. Only the 30-minute arm cleared it for negative mood. So the benefits are real but modest, and they shrink as breaks get further apart.

Does this show movement breaks improve health?

Not directly, and the distinction matters. This trial measured fatigue and mood over two weeks, all self-reported. It did not measure blood sugar, blood pressure, weight or anything else physical, and two weeks is far too short for those outcomes anyway. What it establishes is that people can do this and feel better for it, which is a precondition for health benefits rather than evidence of them.
