Viscous soluble fiber eased constipation on the strongest evidence in 17 trials of 1,423 adults

“Eat more fiber” is the most-repeated piece of digestive advice in medicine, and as an instruction it is close to useless, because it treats one word as if it named one substance.

A network meta-analysis, the method that ranks treatments never tested head to head by chaining them through a shared comparison, does the work that advice skips. The one published in Food & Function pooled 17 randomized controlled trials involving 1,423 adults with chronic idiopathic constipation and ranked fiber subtypes against each other, and the two families a supermarket sells side by side came out doing different jobs.

Viscous soluble fibers were associated with greater efficacy compared with placebo in relieving defecation difficulty, at high certainty. In a literature this messy, that grade is the finding.

Two fibers, two mechanisms

Fiber is a substance in plants, and you may see it listed on a food label as soluble fiber or insoluble fiber. The label treats them as a pair. Physiologically they are barely related.

The viscous soluble kind dissolves and forms a gel. Psyllium is the household example, along with the beta-glucan in oats and barley, and the gel holds water inside the stool, softening it and making it easier to pass. The insoluble kind does not dissolve at all. Wheat bran and the stringy parts of vegetables pass through largely intact, adding bulk and stimulating the gut wall mechanically. Both types have important health benefits, and neither substitutes for the other.

What high certainty means in a field like this

Grading systems exist because effect sizes lie on their own. A large effect measured badly across three small trials is worth less than a small effect measured well across fifteen, and certainty grades are how a review says which it has.

Most nutrition meta-analyses top out at low or very low. This one reports high certainty for one outcome, difficulty passing stool, meaning further research is unlikely to change the estimate much. Stool consistency and stool frequency followed at moderate certainty, which is still unusually strong. The paper’s other label, class 1, is a second layer of the same idea, marking results the reviewers regard as best supported by the network as a whole.

The ranking that looks better than it is

Insoluble dietary fibers showed the highest ranking probability for improvement in overall constipation symptom scores; however, this finding was supported by very low certainty evidence.

Read only the first half of that sentence and insoluble fiber wins the paper. Read the second half and the win evaporates. Very low certainty means the estimate could move in any direction with one more trial, and a ranking built on very low certainty evidence is closer to a hypothesis than a result.

Every ranking of this kind sets the same trap. The output looks like a league table, tables invite you to read down them, and the grade that qualifies each row sits somewhere else entirely.

Which symptom you have decides which fiber

The useful structure here is that constipation is not one complaint, and the paper is organized accordingly. Constipation means having fewer than three bowel movements a week, and the stool can be hard and dry, which already contains two separate problems: how often, and how hard.

Viscous soluble fibers tended to have a higher probability of favorable performance compared with other dietary fiber subtypes in improving straining during defecation, stool frequency, and stool consistency. Straining carries moderate certainty; the other two are low.

Tolerability usually settles the question in practice, and here it points the same way: viscous soluble fiber was associated with a relatively favorable tolerability profile. Bran taken in quantity is famous for gas and bloating, and a supplement people abandon in week two treats nothing.

What to put in the trolley

For difficulty passing stool, the evidence points at viscous soluble fiber, and psyllium is the cheapest and most-studied form of it. That is the one recommendation in this paper that rests on high-certainty evidence.

Whatever you pick, the pace matters. Add it to your diet slowly, because increasing dietary fiber too quickly can lead to gas, bloating, and cramps, and the people who quit fiber supplements are almost always the people who started at a full dose.

One boundary is worth stating plainly. Almost everyone gets constipated at some time, and it usually lasts a short time and is not serious, but a change in your usual pattern is a different thing from a bad week. If your bowel habits change, check with your provider. No fiber subtype ranking substitutes for that.
