Alcohol raises cancer risk even below one drink a day, review finds
Researchers at the Institute for Health Metrics and Evaluation re-analyzed 843 studies covering 20 health conditions. Alcohol was tied to higher risk for about ten cancers at any level, while its links to heart and metabolic disease were mixed. Because the studies were observational, they show associations, not proof.

Alcohol advice has been shifting for years. Health agencies keep tightening their guidance, while the older idea that a daily glass of wine is good for the heart has been slow to fade. A new analysis from the Institute for Health Metrics and Evaluation at the University of Washington set out to map where the evidence is strong and where it is weak, and on cancer its answer is blunt: the risk climbs with any amount you drink.
The study, published in the journal Nature Health, pooled 843 cohort and case-control studies and weighed alcohol against 20 health outcomes. These are studies that track people’s drinking and health over time rather than assign it, so they can flag links but not prove cause. The cancer results were consistent. The heart and metabolic results were the complicated part, and they deserve a close look.
The cancer link held at every level
Across the board, the team reported harmful associations between alcohol and all 10 cancers it examined, and the risk rose as people drank more. Even below one standard drink a day, less than 10 grams of pure alcohol, the risk was higher for cancers of the pharynx (the throat), esophagus, breast and liver, among others.
The size of the effect varied by cancer, but the direction did not. Emmanuela Gakidou, the senior author, put it plainly: “For cancer, the evidence is consistent and unambiguous: risk rises with any level of alcohol intake.”
How the researchers weighed it
To build the estimates, the team ran 16 systematic reviews across four databases and applied a method called Burden of Proof, which is built to report the smallest effect the data can support. Xiaochen Dai, the lead author, framed the design this way: “Our framework takes a cautious approach by accounting for differences across studies and reporting the smallest plausible effect supported by the data.”
That caution matters, because the underlying studies were observational. They can show that heavier drinkers developed these cancers more often, but they cannot prove alcohol was the cause.
The heart story is the complicated part
For several conditions that are not cancers, the risk was not the same at every level of drinking. In some cases, low-to-moderate drinking was tied to lower risk than not drinking at all, while heavier drinking was tied to higher risk. Type 2 diabetes and Alzheimer’s disease each showed a small reduction in risk at lower intakes. For the common forms of heart disease and stroke, any sign of benefit at low-to-moderate intake was inconsistent, and the risk rose at higher levels of drinking.
One pattern held throughout. At high levels, alcohol was tied to increased risk across all outcomes, meaning every condition the team studied. Gakidou cautioned against reading the mixed picture as a reason to drink. “Rather than interpreting these results as an endorsement of drinking, they lay out a complex map of where the evidence is strong, weak, or mixed,” she said.
For a drinker, the evidence splits into two very different messages. On cancer, the bottom line is uncomfortable: the analysis found no amount of drinking it could call safe, and the risk grows the more you drink. On the heart and metabolism, the honest answer is that the evidence is genuinely mixed, and that is a long way from the old promise that a glass a day is good for you.
